Evans Syndrome Complicated by Intratubular Hemoglobin Cast Nephropathy

چکیده
Evans syndrome (ES) is a rare autoimmune disorder whose exact pathophysiology is unknown. It is characterized by the simultaneous or subsequent development of autoimmune hemolytic anemia (AIHA) and immune thrombocytopenia (ITP). Intravascular hemolysis, with hemoglobinemia, is known to produce acute kidney injury; however, the development of intratubular hemoglobin casts (hemoglobin cast nephropathy) in the setting of acute hemolysis is uncommon. Likewise, the association of ES and acute renal failure is equally uncommon. We present a case of a 7-year-old girl with ES who developed acute kidney injury in the setting of intravascular hemolysis and had widespread intratubular hemoglobin casts.

Paraprotein-Related Kidney Disease: Evaluation and Treatment of Myeloma Cast Nephropathy.
Nearly 50% of patients with multiple myeloma develop renal disease, most commonly from AKI caused by cast nephropathy. Development of AKI is associated with poor 1-year survival and reduces the therapeutic options available to patients. There is a great need for more effective therapies. Cast nephropathy is caused by the interaction and aggregation of filtered free light chains and Tamm-Horsfal...
متن کاملResolution of cast nephropathy following free light chain removal by haemodialysis in a patient with multiple myeloma: a case report
INTRODUCTION Acute renal failure in multiple myeloma is most frequently caused by cast nephropathy, when excess monoclonal free light chains co-precipitate with Tamm-Horsfall protein in the distal nephron, causing tubular obstruction. The natural history of cast nephropathy after diagnosis is unknown. This report provides supporting histological evidence that, as serum free light chain concentr...
متن کاملGlomerular haematuria, renal interstitial haemorrhage and acute kidney injury.
Macroscopic haematuria of glomerular origin has been associated with acute kidney injury. We report a patient with IgA nephropathy, macroscopic haematuria and acute kidney injury. Systemic anticoagulation may have aggravated haematuria. There was extensive interstitial and intratubular red blood cell extravasation, and interstitial haemosiderin deposits. The abundant presence of macrophages exp...
متن کاملOverview on Evans syndrome: In the course of Covid-19 pandemic; essentials and approaches
Redundant studies proved coronavirus infection results from a defect in a suitable immune response that may exacerbate immune-inflammatory reactions like cytokine storm and autoimmunity. Evans syndrome (ES) is a rare chronic autoimmune disease that distinguishes it from autoimmune hemolytic anemia (AIHA) and immune thrombocytopenic purpura (ITP). Reports have shown significant differences in im...
متن کاملThe co-occurrence of multiple sclerosis and Evans syndrome: A case report
Background: Evans syndrome is an uncommon autoimmune disorder manifested by fatigue, jaundice, pallor, purpura and petechiae. The main characteristics of this rare disease are simultaneous or sequential existence of positive anti-globulin test, immune thrombocytopenia (ITP) and autoimmune hemolytic anemia (AIHA). Evans syndrome as an autoimmune disorder can be associated with other diseases. Th...
